Outreach

Outreach is a first step in bringing individuals who do not access traditional services into treatment. Effective outreach is aimed at engaging persons into the needed array of services, including identification of individuals in need, screening, development of rapport, offering support while assisting with immediate and basic needs, and referral to appropriate resources.

Homelessness Outreach

This program is designed to provide early case management and substance use services to individuals experiencing homelessness. Services may include case management, motivational counseling, substance use and co-occurring services referral, permanent housing and other support services, and/or transportation to safe locations or service providers (including detoxification or medical care if necessary). All services include support from our peer support specialist team and alumni team!

The case management services provide the following:

  • Coordinating treatment services into all levels of care
  • Facilitating access to appropriate and necessary services
  • Providing linkage between participant needs and services
  • Monitoring participant’s progress for placement
  • Providing social support
  • Promoting treatment or community adjustment
  • Providing advocacy to ensure that services are appropriate to the participant’s needs
  • Providing outreach services necessary to help the participant obtain benefits to which participant is entitled

Substance Use treatment services:

  • Outpatient Level 1.0 and 2.1
  • Individual sessions with co-occurring clinicians
  • Substance Use Counselors
  • Case Managers
  • Education and therapy groups

COVID Outreach

COVID outreach team assists individuals using case management services, prevention services, and substance use/co-occurring treatment services to engage in an enhanced continuum of care for substance use disorder treatment to meet an anticipated increase in need resulting from the COVID-19 pandemic. All services include support from our peer support specialist team and alumni team!

  • Biopsychosocial assessments
  • Coordinating treatment services at all levels of care
  • Coordinating detox services
  • Facilitating access to appropriate and necessary services
  • Providing linkage between participant needs and services
  • Monitoring participant’s progress for placement
  • Providing social support
  • Providing advocacy to ensure that services are appropriate to the participant’s needs
  • COVID relief funds Mat-Su area
  • Medication Assisted Treatment referrals and support

Eligibility

Open to all racial and ethnic backgrounds whom have sporadic living arrangements and or identify as homeless; have been negatively impacted by COVID 19.
